SCHOLARSHIP ATTENDANCE AND DAMAYAN HOURS LOG

I understand that I have read the 2019 Scholarship Rules and acknowledge the eligibility requirements. To qualify, I must have attended at least six (6) General Membership meetings or six (6) 2 hour Damayan service or combination of six (6) total attendance and damayan completed from July to June of the current scholarship year. I also understand that the applicant and/or immediate family members (defined as parents, grandparents, siblings, step-siblings, or legal guardians who are active Filipino Community Inc. members) may perform the Damayan services or attend meetings on behalf of the applicant.
